Alumni Referral Program Survey Questions 2008

The following 20 question survey has been developed to help us receive feedback about the Alumni Referral Program. This feedback will be used as we evaluate the past year and make plans to better the program for the future. Upon completing the survey, you will be entered into a drawing for 2 tickets to the Utah State vs. BYU football game on October 3, 2008 at Romney Stadium. Your e-mail address will be gathered for the drawing but will NOT be correlated with your answers, but only if you complete the entire survey. Please complete this form by April 25, 2008.
