CORPORATE TRAVEL CARD
Diners/MasterCard Corporate Travel Card
Complete the Application and send to the travel office at UMC 2400.
The travel office will call you when your card has arrived. The cardholder needs to bring with them a picture id when picking up the diners card. The User Agreement form will be filled out and signed by the cardholder at the time of pickup.

Requirements
An employee must be a "benefit-eligible" employee to apply for a Corporate Travel Card and the applications must be signed by the applicant and supervisor.
