Documentation Guidelines
Eligibility for DRC programs is based on a review of medical or psycho-educational information. The need for specific services must be outlined in the information provided.
Please refer to the documentation guidelines below. In all cases, information must be comprehensive, current, and include the name, address, and specialty of the professional who conducted the evaluation. The DRC reserves the right to request more information, if needed, to determine eligibility or needed service.
