Booking Guidelines and Forms |
| Booking an event in one of the FAC Performance Venues can be fast and easy. Simply call the CSAPS Scheduling Coordinator at 435-797-3040 to check available dates and times. |
Not all events are appropriate for all FAC Performance Venues. Please contact the CSAPS Scheduling Coordinator for help determining which venue will best meet your event needs. A Preliminary Reservation Request is required for all events held in the FAC Performance Venues. The first step in the booking process, after calling the CSAPS Scheduling Coordinator, is to download the Preliminary Reservation Request, fill it our using Adobe Acrobat Reader, print it out, and mail it to the CSAPS Scheduling Office. The CSAPS Scheduling Coordinator can provide assistance in filling out the Preliminary Reservation Request. Once we have this form on file, your date will be held and the CSAPS Scheduling Coordinator will assist you with the rest of the booking process. |
