Letters of Recommendation
The school of Graduate Studies will notify the recommender upon completion of the application. The recommender will then go to our online recommendation letter and submit the letter electronically.
NOTE: RECOMMENDER NAMES AND E-MAIL ADDRESSES ARE REQUIRED ON THE ONLINE APPLICATION.
These should be used to request three persons to submit letters of recommendation (At least two letters must come from persons who are familiar with you, and can make an authoritative assessment of your recent academic progress and success).
Affidavit of Support for I-20 Documents (International students only)

Assurance of adequate financial support must be submitted before the I-20 document will be issued. This should be in the form of a sponsor letter and bank statement. Students are expected to pay the tuition and fees by the fee payment deadline for each semester.
Split Form and Policy

A Utah State University undergraduate who has received permission to file a Split Form in order to register for graduate courses must be within 30 semester credits of completing bachelor's requirements, have filed an Application for Graduation in the Graduation Office, have a 3.0 or higher GPA, and have applied for admission to the School of Graduate Studies.
