Application

To apply for Intensive English study only

1. Submit these forms: (1) IELI Application Form , (2) Affidavit of Support, and a Bank Statement or a official letter from a sponsor.
2. Prepare money order for a non refundable application fee of $75 (or $90 late fee) and $50 placement fee. The placement fee may also be paid on the day of the exam. To pay by credit card, call +435-797-1124.
3. Official transcripts and certificates or certified true copies for each secondary school, college and university attended with certified English translations of all documents should be sent to International Admissions, Office of Global Engagement, Utah State University, 9545 Old Main Hill, Logan, UT 84322-9545.
4. Please fax or mail a copy of the photo page of your passport to the Office of Global Engagement, International Admissions. We do not recommend that you email the passport copy because of the security risk to your personal information.
