IELI Affidavit of Support and Certification of Funds

Personal or family Please have your guarantor sign the statement below and submit a bank statement to verify that adequate funds are available for your support.
Government or other sponsoring organization Please enclose a signed letter from your organization guaranteeing their financial support.
TOTAL Must meet minimum of $36,150 for one calendar of study for student only. If you are bringing your spouse, add $5,000. For one child, add $2,500. For the second child, add $1,500. For each additional child, add $1,000.
Guarantor's Certification of Financial Support:
I certify I will support this student while he/she studies at Utah State University and that the required funds are available for his/her support. Enclosed is a bank statement verifying that adequate funds are available.

Print out this form and send it with your "IELI application for "English only" along with a official record (transcript) of your grades from your most recent school.

Note: Students applying for "English + undergraduate" or "English + graduate" should use the affidavit forms which come with the undergraduate and graduate application packets.
