Skipping Summer Term?

You do not need to apply for a leave of absence if you plan on attending fall 2008.

If you are registered for Summer 2008 and want to drop your classes then please continue to fill out this Leave of Absence form.

Use this form to:
  • request that all of your classes be dropped


  • request a Leave of Absence from USU if you intend to return to USU in the future (This is a good idea if you are leaving on a mission, on military assignment, or other absence.)


  • request deferment of admission if you are a newly accepted freshman, transfer or readmit student who wishes to defer your start date to a later semester.


  • request that your scholarship be held during an absence


  • withdraw permanently from the university if you do not intend to return

If you are a non-degree seeking student attending a Regional Campus or Distance Education (RCDE) Center, please do not use this form. Instead, please contact your local RCDE Center for more information.

Note: You may take up to a one-year leave, unless you are serving in the military, on a religious or humanitarian mission, or for extenuating circumstances approved by the Retention and First-Year Experience Office.
