Scholarship Acceptance Agreement - Testing Phase
The following are general conditions of scholarship awards at Utah State University. Please read the complete conditions on your Scholarship Acceptance Agreement carefully. The conditions listed on your individual agreement are the conditions that apply to you.
Retaining Your Scholarship
Please see your Scholarship Acceptance Agreement received by mail for important details on how to retain your scholarship, including required credit hours per semester and GPA. If you do not meet these criteria, you will be placed on probation for one semester. If you do not meet the required standard by the end of your probationary semester, you will lose your scholarship. You may appeal the loss of a scholarship through the Financial Aid Office.
Other Conditions
You can only use this award for undergraduate coursework completed at the main campus in Logan. It cannot be used for online, independent study, satellite, or any class not attended at the campus in Logan. You can apply one of your scholarship semesters to a summer semester if you are a Utah resident.
You must use this scholarship during consecutive semesters (excluding summer semester) beginning Fall 2008. Your scholarship can be put on hold for extenuating circumstances such as illness, military duty, or church service. Your scholarship cannot be put on hold to attend another institution of higher education. You must complete a leave of absence form online at http://www.usu.edu/loa to put your scholarship on hold.
You are responsible for making sure your classes are paid in full by fee-payment deadline. Your award only applies to tuition. You are responsible for student body fees, class fees, and other charges. If you have not paid these additional costs by fee-payment deadline, you will lose your classes.
If you are a nonresident of Utah, you are encouraged to gain residency by living in Utah for 12 continuous months and by declaring financial independence. For more information on Utah residency, please visit www.usu.edu/admissions/residency.
Accepting Your Scholarship
In general, you can only accept one tuition scholarship offered through Utah State University. If your tuition is already being covered by another source, contact the Admissions Office at (435) 797-1878 or 800-488-8108 to discuss other options for your scholarship to be applied.
To accept this award, please do one of the following by May 1, 2008:
- Sign and return the Scholarship Acceptance Agreement to the Admissions Office at USU
- Submit this form online
If this form is not submitted by this deadline, we will not be able to honor this scholarship offer. If you have questions, please call us.
