USU/Community Associates Award Public Service Announcement


The USU/Community Associates is seeking nominations for their annual recognition award given to two individuals during their spring social on Thursday, April 15, 2010. Please download and complete the nomination form identifying the individual along with a brief paragraph explaining why this person is deserving of the award and what they have done to benefit either the university or the community or both.

Nominations should be completed and mailed to Jan Appuhn, USU/COMMUNITY ASSOCIATES at Utah State University, Old Main 112, Logan, UT 84322-1440 or emailed to jan.appuhn@usu.edu by Friday, January 8th 2010.
