Order a replacement diploma by mail or in person. We will ship it within 10 business days. If you choose to pick up your Replacement Diploma, you will be notified via email when it is ready to be picked up.

Print and fill out our Diploma Replacement Form and:

• Bring it to the Registrar's Office: TSC (Taggart Student Center) 246 or
• Mail it to the: Utah State University, Registrar's Office, 1600 Old Main Hill, Logan UT 84322-1600

The charge for a replacement diploma is $10 for undergraduate and $15 for graduate. Payment can be made by cash, check, or money order.
