Records and Transcripts

Due to the recent data conversion, Utah State University must review your transcript before it can be released as an Official Transcript. Therefore, there is a delay of up to 10 business days from the time you order your transcript until it is available to be mailed or picked up. We appreciate your patience and remind you to plan ahead.

Ordering an Official Transcript (in person)

In order to process your request the following information is needed:

  • Transcript Request Form
  • Full name and any other names attended under
  • Student A#
  • Method of payment (Check, Money Order, or Cash)
  • Date of Birth
  • Dates of Attendance
  • Your Signature is Required by Federal Law
  • Provide the complete address of where to send the transcript
  • Fee Payment $2 per copy
  • Fax Fee Payment $5 per copy

Please note that all fees must be paid before a transcript is mailed or faxed.

Ordering an Official Transcript in Person

The Family Education Rights and Privacy Act (FERPA) requires the school to have a written release from the student before issuing transcripts.

Transcripts will go to the address(es) provided by the student. Please make sure that you provide the complete and accurate address(es).

If you have recently completed a degree or course and your transcript must the degree or course posted before it is sent, please not that on your request.

USU accepts MasterCard or Discover Card for credit card transcript fee payment online via ACCESS only. American Express and VISA card are NOT accepted.

If you would like your transcripts sent by fax, the fee is $5. Faxed transcripts, however, do not transmit clearly and are barely legible due to the security paper we use. Also, many colleges and universities do not consider a faxed transcript 'official.' When ordering a transcript to be faxed, please include a name and fax number where you want the transcript sent.

IMPORTANT:

No transcript will be sent until all financial obligations with Utah State University are met.
