Standard Enrollment Verification
Standard enrollment verification certificates can be generated instantly from the National Student Clearinghouse. This option may not meet the needs of every requester or organization, but in many instances it does. It's fast, easy, and free!
To download an official enrollment verification from the National Student Clearinghouse, follow these steps:
- Visit this link: https://ss.banner.usu.edu/ssomanager/c/SSB?pkg=Z_ClearinghouseLink.p_clr2go1
- Select Connect to Clearinghouse.
- Select Current Enrollment (for the current semester) or All Enrollment (for all semesters).
- Click the link Obtain and enrollment certificate. Depending on which option you selected, the certificate can take a few moments to load.
- Your certificate will be displayed and can be downloaded to be used as an official enrollment verification from Utah State University.
This verification contains your enrollment status (Full Time, Half Time, Less Than Half Time) for all terms enrolled at Utah State University. This document should not be used for loan deferment verification purposes. Those verifications are handled by lenders using online access to the Clearinghouse. Refer lenders with questions to firstname.lastname@example.org.
Complete Academic Verification
A verification is an official document that confirms attendance, awarded degrees, current and past enrollment, expected graduation date, available test scores, cumulative GPA, and other parts of a student record. Official verifications include the University seal and the Registrar's signature.
Students may need verification of enrollment to defer student loans, to prove enrollment for their health insurance, to verify degrees for prospective employers or to show proof of academic standing to enroll in another institution. Verification of directory information may be requested without a written release from the student. Utah State University defines directory information as local and permanent addresses, dates of attendance, enrollment status, school and major field of study, classification, earned degrees, expected degree date, and similar information.
The University partners with the National Student Clearinghouse for deferment of student loans. If your servicer is requesting your enrollment status, refer them to email@example.com. If your servicer needs a deferment form processed, bring the completed form to the Registrar's Office. Write the semester for which you are requesting deferment in pencil on top of the form. We will forward the form to the Clearinghouse, who will confirm your enrollment with your servicer.
Certain confidential information may be released only with the student's written authorization and signature, in accordance with the Family Educational Rights and Privacy Act of 1974.
Request complete academic verification online.
Certifications may be ordered online by mail, by fax, by telephone, or in person. There is no charge for official certifications to be picked up the next day, or to be delivered by first class mail. A $5.00 charge applies for same day delivery, notarization, or delivery by FedEx. (Additional charges for mailing via FedEx apply). These can be paid by cash or check or online if you have access to your Banner account. Students who wish to request a certification of non-directory information can submit their request.

Order by mail. A request for certification must include the following information. Mail the form or letter to Utah State University, Enrollment Certification, 1600 Old Main Hill, Logan UT 84322-1600.
- The date of the request
- Student's full name as it appears on University records
- Student's A-number and date of birth
- The type of information you are requesting to be printed on the certification
- The purpose behind your request
- The name and address to whom it should be mailed
- The number of certifications requested for each address
- Student's signature required for all requests
- Postal address of requesting party, email address and daytime phone
- Any special instructions
- Any forms to be attached
Order by fax. Fax a letter addressed to the Utah State University, Enrollment Certification, which includes the information as above. Requests may be faxed to 435 797-1110.
Order by telephone. Certification of directory information may be requested by calling 435 797-1116 Monday through Friday between 8am and 5pm.
Order by email. Certification of directory information may be requested by email at firstname.lastname@example.org. Please attach a scanned document with the requested information along with signature. Provide the student's full name, A-number and date of birth.
Order in person. Come to the Registrar's Office, TSC 246, Monday through Friday between 8 am and 5 pm. You must present a photo ID when you order. A mailing address is required if the certification is to be sent to a third party.
Degrees and Attendance Verification
The National Student Clearinghouse at degreeverify.org provides dates of attendance and earned degrees to employers and background screening firms.
Our website provides public information for all students enrolled since 1980. For students enrolled before then, call the Records Department in the Registrar's Office at 435 797-1116.
