Scholarship Acceptance Agreement
The following are general conditions of scholarship awards at Utah State University. Please read the complete conditions on your Scholarship Acceptance Agreement carefully. The conditions listed on your individual agreement are the conditions that apply to you.
To Accept Your Scholarship
- Initial each section, sign, and date your agreement.
- Save your document as a .pdf .jpg .png .tif document and ensure it is readable after saving.
- Your document name should be your A# (ie A00000000.pdf). If you don’t know your A#, save the document with your full name.
- Upload your document by the acceptance deadline, which is indicated on your agreement. Agreements received after the deadline will not be accepted or honored.
Retaining Your Scholarship
Please see your Scholarship Acceptance Agreement received by mail for important details on how to retain your scholarship, including required credit hours per semester and GPA. If you do not meet these criteria, you will be placed on probation for one semester. If you do not meet the required standard by the end of your probationary semester, you will lose your scholarship. You may appeal the loss of a scholarship through the Financial Aid Office.
Accepting Your Scholarship
In general, you can only accept one tuition scholarship offered through Utah State University. If your tuition is already being covered by another source, contact the Admissions Office at 435-797-1079 or 800-488-8108 to discuss other options for your scholarship to be applied.
Ensure your document can be opened and read before uploading. Documents that are not in focus, that don’t include the entire page, or that can’t be opened will not be accepted.
