How to Apply

  1. Read through the eligibility requirements (below).
  2. Check the Eligibility Estimator to determine whether your family is eligible.
  3. Collect the required documents for your application.
  4. If not enrolled in childcare, contact a childcare provider from the CCAMPIS network of providers to reserve a spot for your child. If you have a provider outside of the CCAMPIS network, gather contact information for your current provider. If you haven't yet selected a provider, visit the Care About Childcare website to find a state-licensed provider with openings. Applications are not considered complete until a provider has been selected.
  5. If you are employed, consider applying for Department of Workforce Services (DWS) childcare subsidies in addition to CCAMPIS. DWS childcare support is similar to CCAMPIS and can be used with CCAMPIS to cover up to 100% of care. Learn more at the DWS childcare website and apply.

You may begin an application even if you don’t have all the documentation. Use the “Save & Return Later” button at the bottom of the page to continue your application at a later date. If you need assistance with your application, contact the CCAMPIS Project Manager.

Eligibility

USU-CCAMPIS subsidies help pay for childcare while students attend classes and complete course-related activities. For children in kindergarten and younger, subsidies can cover care up to full time. For children aged 8 or younger, subsidies can cover part-time before/after school care. Care must be provided by state-licensed child care providers. Additional information about eligible providers is available on the CCAMPIS Providers page. 

To be eligible for a CCAMPIS subsidy, at least one household member
must meet the following requirements:

Student at USU - Logan, Brigham City, Eastern, or Blanding/Montezuma Creek

Students must be enrolled at USU for the semester for which they apply. Because the number of enrolled credit hours influences subsidy levels, CCAMPIS enrollment follows USU’s registration schedule. The CCAMPIS subsidy application for the upcoming semester opens when USU course registration begins. CCAMPIS uses the A number(s) listed in your application to view your transcripts and determine eligibility each semester.

Under the terms of the grant that funds CCAMPIS, subsidies can be offered to students attending USU at the Logan, Brigham City, Eastern (Price), or Blanding/Montezuma Creek campus and receiving childcare from a qualified provider in the corresponding community. 

In Good Standing

An undergraduate student is considered by the university to be in good standing when s/he earns a 2.00 or higher semester gpa and the institution gpa is higher than 2.00.

Pell Grant Eligible

Due to the nature of CCAMPIS funding, subsidies are prioritized for Pell-grant eligible students. For USU to determine Pell Grant eligibility, you must complete a Federal Student Aid (FAFSA) application. Pell eligibility will be reported on your account summary in Banner; instructions for acquiring Pell Grant documentation from Banner are on the Required Documents page. Low income foreign undergraduate students who do not qualify for federal aid may apply for CCAMPIS assistance if they meet all other eligibility requirements.

If funding remains after subsidies have been awarded to all qualified Pell grant eligible students, applications from students who meet other requirements but are not Pell-grant eligible may be considered for funding. Doctoral students and students completing second Bachelor's degrees are not eligible for CCAMPIS subsidies. 

Low Income

CCAMPIS is intended to serve low-income families. Tables from the Department of Workforce Services are used to determine low income eligibility based on household size and income. To verify monthly income, applicants must provide the first page of the previous year’s income tax return showing adjusted gross income or monthly pay stubs if no taxes were filed. If your household does not have income to report, please complete and upload this form with your application.
