Fingerprinting

for child care programs in Cache, Box Elder and Rich Counties and USU students

After submitting a background check, all caregivers working in licensed programs must complete a fingerprint scan.

We will do our best to serve license exempt and other programs but staff time may be limited. Other locations that provide Live Scan Fingerprints Locations.

Fingerprinting is by appointment only, walk ins cannot be accomodated.

Scheduling an appointment

1. Please complete your background check form before scheduling an appointment.

Because this service is staffed by USU students, appointment times will depend on student availability but should be most weekdays during business hours.

Photo ID: Bring a current government-issued photo ID (Driver’s license, passport, or military ID). We cannot complete your appointment without it.
Fingerprint Processing Form: Print and fully complete the Fingerprint Processing Form before your appointment. If you can’t print it, we’ll provide one when you arrive.

Each appointment lasts about 10–20 minutes.
Arrive on time. If you are more than five minutes late or don’t have proper ID, your appointment will be rescheduled.
If you feel sick, please reschedule for a later date.
For any other changes, contact us ahead of time to reschedule or cancel. No shows will need to contact another Other locations that provide Live Scan Fingerprints Locations and there may be a cost.
