Report to the University
The University endeavors to respond promptly when incidents of discrimination or sexual misconduct are reported to the Civil Rights & Title IX Office, including the Title IX Coordinator, or when incidents of sexual misconduct are reported to a Reporting Employee.
When the Civil Rights & Title IX Office receives an incident report, it will contact the person who experienced the sexual misconduct or discrimination (the Claimant) to provide information about reporting options, supportive measures, designated confidential resources, and the Civil Rights & Title IX Office grievance process.

Privacy of Information Shared
The Civil Rights & Title IX Office and any other Reporting Employee(s) who receives a disclosure of Sexual Misconduct will respect and safeguard the privacy of the individual(s) involved. The Civil Rights & Title IX Office will only disclose information as necessary to provide supportive measures, to investigate a formal complaint, when needed to ensure a non-discriminatory environment and/or the safety of campus and campus community members, or to otherwise meet its responsibilities under state and federal law, and USU’s policies and procedures.
The Civil Rights & Title IX Office will respect and recognize a Claimant’s request that their name or report not be disclosed to a Respondent unless the Civil Rights & Title IX Office concludes that it must move forward with a university-driven investigation or another action in order to provide a safe, non-discriminatory environment for all Students, Employees, and the campus community, including the Claimant.
Designated Confidential Resources and other off-campus services are available to speak confidentially and provide confidential services.
Anonymous Reports
Reports about discrimination and sexual misconduct can be made anonymously. However, anonymous reports do not always contain enough information to pursue an investigation that can lead to disciplinary action.
Reporting Employees cannot report anonymously and are required to include their name when filing a report about sexual misconduct.
Amnesty for Student Code Violations
USU offers amnesty for drug and alcohol policy violations of the Student Code when a student reports an incident of sexual misconduct, whether they experienced or witnessed it. For example, if you experience or witness sexual assault and report it to the Title IX Coordinator, you will not get in trouble for a student code violation such as underage drinking or having alcohol on campus. Students who participate as a witness in a sexual misconduct grievance process are also granted amnesty, though they may be encouraged to participate in health education.
Additional Reports
In certain circumstances, it may be appropriate to file reports with additional offices.
If you would like to report a student’s behavior because it disrupts a classroom or university activity or if there may be a threat of harm to oneself or others, please file a Student of Concern report.
If you are aware of circumstances involving individuals who are engaging in concerning or threatening behavior that may pose a threat of violence to themselves or members of the University Community, please file a report with the Threat Assessment Management Team.
If you believe a child or vulnerable adult is being abused or neglected, you are required to report it to the Department of Family and Protective Services (DCFS), the local police (911 or a non-emergency line), or the Utah Abuse/Neglect Hotline 1-855-323-3237. Known or potential child abuse should also be reported to USU’s Youth Protection program.

Report an incident involving an alleged violation of interim USU Sexual Misconduct Policy 2102 or 2103, which include: sexual harassment, relationship violence, sexual assault, sexual exploitation, and sex-based stalking. You can file a report on your own behalf or on behalf of anyone who may have experienced sexual misconduct. Report Discrimination
Report an incident of discrimination or harassment based on protected categories of people including: race, color, religion, sex, gender identity or expression, sexual orientation, national origin, age (40 and older), disability, genetic information, or status as a protected veteran ("Protected Characteristics"). You can file a report on your own behalf or on behalf of anyone who may have experienced discrimination.
Report Sexual Misconduct
According to interim USU Policy 3012, Reporting Employees are required to report an incident involving an alleged violation of interim USU Sexual Misconduct Policies 2102 or 2103, which include: sexual harassment, relationship violence, sexual assault, sexual exploitation, and sex-based stalking.
