Short Term Disability (STD) insurance can replace part of your income if a qualifying disability keeps you from working for a short time (120 days or less). It pays 66.67% of your pre-disability earnings, up to $1,500 per week.

Who is Eligible for Short-Term Disability?

All benefit-eligible employees qualify for short-term disability. USU pays a portion of the premium, so you only pay $2.25 semi-monthly. If you do not enroll in short-term disability as a new employee, you will have another opportunity during open enrollment. Enrolling during open enrollment means you will be subject to an Extended Benefit Waiting period of 60 days during the first 12 months of coverage for a disability caused by physical disease, pregnancy, or mental disorder.

Benefit Waiting Period

If you suffer a qualifying disability, your benefit waiting period is the length of time you must be continuously disabled before you can begin receiving your weekly benefit. The benefit waiting period is 14 calendar days or 10 work days for accidental injury, physical disease, pregnancy, or mental disorder.

How Do I Use Short-Term Disability?

  1. Notify Human Resources
    • Call the Human Resources Solution Center at 435-797-0122.
    • Express your interest in filing an STD claim.
    • Schedule a meeting with HR. They can accommodate in person, via Zoom, or by phone to assist you.
  2. Notify Your Supervisor
    • Inform your supervisor of your need for leave and your intention to file an FMLA request & STD claim.

  4. Short-Term Disability Claim Process
    • To file a claim by telephone, contact The Standard’s Claim Intake Service Center at 1-800-378-2395. (This offers the quickest response time.)
    • A typical application for disability benefits contains the following documents:
      • Employee's Statement
      • Employer's Statement
      • Attending Physician's Statement (APS)
      • Authorization to Obtain and Release Information
    • When I report my claim, what information will I need to provide? You will be asked to provide the following information - in addition to other questions about your absence:
      • Employer Name: Utah State University
      • Group Policy Number: 762781
      • Name and Social Security Number
      • Last day you were at work
      • Nature of claim/medical information
      • Physician's Contact information (name, address, phone, and fax number)
  5. Communicate your Plans
    • Inform HR and your direct Supervisor of your leave plans.
    • Enter your leave details in the USU timekeeping systems.

      • AIM (facilities employees working on the Logan campus)
    • Human Resources can guide you on how to enter your leave properly. It can be confusing with both FMLA and STD running concurrently.

By following these steps carefully will help ensure your leave and benefit claims are processed smoothly and in accordance with university policy.
