To schedule an appointment, please call (435) 623-0537 or email maren.kaldor@usu.edu at least 24 hours in advance. Walk-in tests are not allowed. Please note the following:
- Picture identification is required.
- No backpacks, phone, etc. allowed in the testing area.
- Non-USU students will be charged a $10 fee per test. It is your responsibility to include information we need to access non-USU tests. Make checks payable to Juab School District.
- No children, friends, or family allowed.
- When making appointments please include:
- Class name
- Class Type (Online/Broadcast)
- Instructor's Name
- Your name and phone number
