USU Students
- A Scheduled Appointment is required for all Exams.
- Walk-in tests are not allowed.
- Scheduling testing appointments online is only available 24 or more hours in advanced. USU students can schedule their test online at: rcde.usu.edu/mats. Less than 24 hours? You will need to call the USU Orem Testing Center to check testing availability: (435) 797-5374
- Tests must be started at least 1 hour prior to the closing of the testing center.
- Testers more than 30 minutes late for their testing appointments will have their appointment cancelled.
Non USU Students
- Non-USU students need to call (435) 797-5374 at least 24 hours in advance to schedule any testing appointments
- A fee of $20.00 is charged for Proctoring per test (payable by credit card or exact cash amount).
- If this is your first time testing at USU Orem please arrange proctoring at least a week before your expected first test date. You will be required to contact your institutions testing center or Professor and make the arrangements for them to contact us with your testing information before we will be able to schedule your exams.
- Please be prepared to provide the following information when scheduling a test:
- Name
- University or Institution
- Phone Number
- Class (i.e. BIOL 1010)
- Exam Name/Number (i.e. Midterm, or Exam #1)

(We are in a business complex called “Central Park”, we are in the “West” Building. From State street you will see a big red brick building with a sign that says “InfoTrax” that is the building we are in.)
Testing Center Policies and Expectations
- Make sure you have a valid photo ID ready. We cannot administer any exam without positive identification (USU ID, driver license, state ID, passport, Military ID).
- All cell phones and electronics must be powered off and put away prior to receiving a test. *Any prohibited use of cell phones or electronics during an exam will result in an incident report that will be sent to your instructor.
- Backpacks, purses, laptops are not allowed in the testing center.
- Students are only allowed to have items on their desk that are named by their instructor. Purses, wallets, or other sundry items are not allowed on the desk.
- The only talking allowed in the test room must be directed toward a proctor.
- Any issues involving a test's instructions or availability are to be brought up with the instructor. The Testing Center is not a liaison or arbiter between the student and instructor.
- Children are not allowed to accompany a parent into the testing room. We never take custody or responsibility to babysit. This brings liability upon the University. Children in the waiting area are to be attended to at all times, thus students must make prior arrangements to care for their children.
- No notes / scratch paper are allowed to leave the testing center ever.
- Watches, of all makes and models, are not to be worn by any test taker.
- If you have any writing on yourself, do your best to remove it prior to testing. If the proctor sees any markings on you it will be investigated and your instructor notified.
- We reserve the right to investigate any suspicious behavior.
- The consumption of food is not allowed during testing (unless needed for medical reasons only). Drink containers that have a straw or do not have a twist on lid are not allowed to enter the rooms.
- Bathroom breaks are not allowed. (Unless needed for medical reasons only).
- Hats and beanies must be removed.
- Hoods of any type may not be worn on top of your head while testing.
- Sunglasses may not be worn while testing.
- You may raise your hand to ask the proctor a question and they will approach you; however our proctors are not allowed to clarify any test material or help answer test questions in any way.
- The only testing aids that will be permitted are ones specified to you at check-in. If you have any questions or concerns regarding testing aids make sure to ask before testing. Please note that it is up to your instructor to inform us (in MATS) of any testing aids permitted. Emails from an Instructor or showing of Syllabi will not be accepted as acceptable Instructor permission.

Math Placement Exam (ALEKS)
- To take a Proctored Math Placement Exam please call 435.797.5374 (you can not schedule a math placement exam online, sorry).
- Please log in to your ALEK account at least once before coming to test.

- There is no fee for this exam if you are an USU student.
Food Safety Managers Exam
