UATP in Salt Lake City (formerly CReATE)

In Salt Lake City, the Utah Assistive Technology Program specializes in mobility devices that are donated, refurbished and transferred back to people who need them for a modest service fee.

Service fees for manual devices range from $35 to $150 and power devices from $250 to $580.  
No insurance or proof of disability is required to receive a device. The program partners with an Assistive Technology Professional (ATP) and an Occupational Therapist as needed to do mobility evaluations to ensure individuals receive appropriate equipment.

A funding specialist is also available through a partner agency to help those who may not be able to afford a mobility device.

Tuesday 8 a.m. to 5 p.m.
Wednesday by appointment only
Thursday 8 a.m. to 5 p.m.

Our workload is consistently very heavy, and we may be out of the shop or we simply may not be able to give you the attention you deserve without an appointment. Thanks!

Transferring Device Ownership

UATP Salt Lake does not loan or rent equipment. The ownership of the device will be transferred from UATP to the individual acquiring the AT device. UATP’s intent is to transfer AT devices to individuals, and therefore the devices are not for resale. Service fees are flat rate fees.

How to Donate a Device or In-kind Item


  1. Contact us to determine if the device can be accepted.
  2. Deliver the device.
In addition to used mobility equipment, the warehouse needs items to refurbish chairs and keep the operation running.

Parts and chairs unable to be used are recycled with reputable area recycling companies. The money earned is put back into the AT reuse program in Salt Lake City.

 

Who We Serve


Services are provided to people with disabilities throughout the state of Utah. UATP will accept consumers regardless of race, national origin, religion, geographic location within the state of Utah, age, sex, disability, political affiliation, sexual orientation or belief. No person shall be excluded from participation in, be denied the benefits of, or be subjected to discrimination by UATP. The UATP staff reserves the right to deny services to any consumer when the severity of the disability or complexity of the technology needs of the individual is beyond the scope and training of the staff. Device transfer services are limited to refurbished devices in the current inventory.

Volunteering in the SLC Location


If you are even a little mechanically inclined and are willing to lend a hand, UATP needs you! We are always looking for volunteers who can help out on an on-going basis. At this time, we do not offer one-time volunteer opportunities. To find out more, contact Dan O'Crowley.
