Information - please read everything on this page

After submitting a background check, all caregivers working in licensed programs must complete a fingerprint scan.

The Child Care Resource Agency offers no cost fingerprinting for caregivers working in licensed programs in Cache, Rich, and Box Elder Counties.

We will do our best to serve license exempt and other programs but our staff time may be limited.   Other locations that provide Live Scan Fingerprints Locations. 

Fingerprinting is by appointment only, walk ins cannot be accomodated.

The $35 background check fee will be billed to your employing program by Child Care Licensing after you submit your form.

Scheduling an appointment

1. Please complete your background check form before scheduling an appointment.
2. To schedule or ask questions, contact us at 435-797-1552 or childcare.help@usu.edu.
3. After scheduling, you’ll receive a confirmation by text or email with:

• Your appointment date and time
• A link to our fingerprinting page for directions to our office and parking instructions

• Monday–Thursday: 9:30 AM – 4:00 PM (appointments only - walk ins cannot be accomodated)

What to Bring to Your Appointment

• Photo ID: Bring a current government-issued photo ID (Driver’s license, passport, or military ID). We cannot complete your appointment without it.
• Fingerprint Processing Form: Print and fully complete the Fingerprint Processing Form before your appointment. If you can’t print it, we’ll provide one when you arrive.

Appointment Details

• Each appointment lasts about 10–20 minutes.
• Arrive on time. If you are more than five minutes late or don’t have proper ID, your appointment will be rescheduled.
• If you feel sick, please reschedule for a later date.
• For any other changes, contact us ahead of time to reschedule or cancel.  No shows will need to contact another Other locations that provide Live Scan Fingerprints Locations and there may be a cost.
