Why was I selected for Verification?

The U.S. Department of Education routinely selects some FAFSA applications to check that the information is accurate and to discourage fraud. If you are selected, you (and your parent, if required) must complete online forms and submit documents to USU to confirm the information on your FAFSA. Some students are required to verify their identity. If the information in your documents does not match your FAFSA, your federal aid eligibility may change.

You cannot receive financial aid until USU reviews and approves your documents. If you are selected for verification later in the school year and do not submit the required documents, your financial aid may be delayed, placed on hold, or canceled.

How to Complete Verification

The U.S. Department of Education uses a randomized process to verify the information submitted on the FAFSA. You may be asked to fill out a web form and submit various documents to the school in order to authenticate the information submitted on your FAFSA. If you are selected for verification you cannot receive financial aid until the verification is completed.

Beware of Verification Deadlines

Federal aid is time sensitive so make sure you know when deadlines are.

Check your Email (and phone)

We email students about verification selection the same day we receive their FAFSA application. After you register for classes, emails are sent to your USU account. You can opt into text alerts on studentforms to receive notifications about your verification requirements, we also send emails to follow up.

Check your Student Account

If you didn’t receive an email you can see if you are verified on your financial aid Dashboard. Look for a task called Submit Required FAFSA Documentation (USU StudentForms).

Log into StudentForms

If you have a verification task, log into usu.studentforms.com to get started. You must log in with the same name, birthdate, and SSN you used for FAFSA.

Review and Complete any tasks

The StudentForms dashboard will help you get started.

Accept Your Aid

After verification has been completed your financial award will be updated and offered/re-offered to you through your student portal.
