Lyric Theatre Camp Participant Registration & Consent

ALL three steps must be done for registration to be complete. Failure to complete these steps will result in your child being unable to participate in the camp.

Step 1: Register & Pay for the Camp

You register and pay for the camp through our ticketing system. Once you've registered and paid each of your students, you will need to come back to this page and continue with Steps 2-3.

Step 2: Download & Complete the Participant Waiver/Consent Form

This step should only be completed after Step 1 (Register & Pay for the Camp) has been done.

• Complete the Lyric Theatre Camp Participant Waiver/Consent Form for each of your participants (it’s a fillable pdf form)
• Save and name the form with the participant’s name (firstname-lastname.pdf)
• You will need to complete one form for each participant
• Continue to Step 3 and upload the completed form(s) (you can upload all registration/consent forms together)
