Utah State University has transitioned incident and risk reporting from ServiceNow to Origami Risk, the University's new risk management platform. Origami provides a secure, centralized way to report incidents, identify risk trends, and support appropriate follow-up across campus.

Please select the report below that best matches your situation. Each form includes guidance to help ensure your report is reviewed and routed appropriately.

For USU Employees

House

Employee Injury / Illness Report

Use this form to report a work-related injury or illness involving a USU employee or volunteer.

  • Required for all workplace injuries and illnesses
  • Initiates workers' compensation review and follow-up
  • Triggers supervisor and safety review when applicable
Car

Vehicle Accident Report

Use this form if you were involved in an accident while operating a USU-owned or leased vehicle.

Important reminders:

  • Always contact law enforcement following a vehicle accident, even if no other vehicles are involved
  • Take photos of all vehicle damage and upload them with your report
  • Use the driver's contact information when completing the form

Vehicle accident reports are reviewed by Risk Management and Claims.

Building

USU Property Damage or Loss Report

Use this form to report damage to or loss of USU-owned property, including buildings, equipment, or other university assets.

  • Commonly used by Facilities, departments, or employees
  • Used to document damage, assess exposure, and support claims when needed

Other Incident Reports

Injured Person

Non-Employee Injury Report

Use this form to report a physical injury to a person who is not a USU employee or volunteer.

Examples include:

  • Students
  • Visitors or patrons
  • Event participants
  • Members of the public

This form may be submitted by the injured person or by someone reporting on their behalf. A USU employee number or affiliation is not required.

Box

Personal Property Incident Report

Use this form to report damage to personal or non-USU property, where no injury to a person occurred.

Examples include:

  • Damage to a visitor's vehicle or belongings
  • Property damaged during a university event or activity

This report focuses on property damage, not injuries.

Warning

Health and Safety Concern

Use this form to report any other safety concern or potential risk that does not fit one of the categories above.

This may include:

  • Unsafe conditions
  • Near-misses
  • Patterns or emerging risks
  • Concerns raised by employees, students, volunteers, or visitors

These reports help Risk Management and campus partners identify trends and address potential exposures early.

About the Transition to Origami Risk

Origami Risk serves as the University's risk management infrastructure. It provides a centralized place to gather incident information, identify risk drivers, and support reporting and analysis. Origami does not make decisions or determine outcomes; it aggregates signals so that appropriate people and departments can take action when needed.

This transition allows USU to:

  • Improve clarity and consistency in incident reporting
  • Allow public reporting where appropriate
  • Reduce duplicate or misdirected reports
  • Better identify trends and emerging risks over time
