Hazard & Administration Fee Assessment

The Utah Veterinary Diagnostic Laboratory may charge a hazard or administration fee if any of the following conditions are noted upon arrival of specimens at the UVDL facility.

Hazardous Fee Assessment

Submissions will be considered hazardous and be assessed a $30 hazardous fee if they meet one or more of the conditions listed below.

  • The submission contains broken glass.
  • The submission contains sample containers that are open.
  • The submission contains sample containers where a majority of the external surfaces are damp with hazardous fluids, such as formalin, blood, serum, milk, sample media, etc.
    • Melting ice packs are not considered hazardous.
  • The submission contains sample containers where a majority of the external surfaces are coated with fecal material.
  • The external container of the submission (e.g. cardboard box) shows external leakage of hazardous fluids, such as formalin, blood, used sample reagent, etc.
  • The submission form sent with the submission is largely contaminated with hazardous fluids, such as formalin, blood, serum, milk, sample media, etc.
  • The submission contains used hypodermic needles.
  • The submission contains postmortem specimens that are not confined in at least one sealed bag.
  • Delivery of the submission was denied by USPS or other carrier service due to biohazard related reasons, and the UVDL staff had to retrieve the package.

Administration Fee Assessment

Submissions will be assessed a $30 administration fee if they meet one or more of the conditions listed below:

  • The submission does not contain a submission form. One submission form cannot be submitted for multiple owners unless pre-arranged with the UVDL.
  • The submission has greater than thirty (30) samples and does not contain a list of submitted samples.
  • The submission contains samples in which three (3) or more specimens have missing or illegible identification, e.g., due to condensation from ice packs, etc.
