Submission Forms

Please download and fill out the relevant form below, please fill out the submission form completely including the following information:

• Please be sure to indicate on the accession form how you would like results reported to you: fax, mail, or email or a combination of those. 
• Owner’s, animal, and specimen information is needed
• Send an Excel spread sheet if submitting more than 20 samples uvdl@usu.edu
• Provide a submission form for each owner
• Place submission forms with correct samples
