Airgas Receiving and Returns

Fill out this form whenever you (1) order a cylinder, (2) receive a cylinder, (3) put a cylinder in the empty cage to be picked up, or (4) a cylinder is picked up. If you received or returned multiple types of cylinders from airgas, you will have to fill out the form for each type (in other words, you have to fill it out once for a 300 cubic foot UHP helium and again for a 200 cubic foot industrial oxygen).  You only need to use this form for Airgas cylinders.
