• Please write the date in the appropriate box in the spreadsheet below after you have completed the task. 
• Yellow boxes indicate weeks for which you need to complete the tasks and write the date of completion in the box.
• Fill in boxes on the week the tasks are due, not the week the tasks were completed. 
• Can and DNPH tasks are listed on the weeks cans and DNPH are collected from field sites.
