Uploading Forms
Before You Upload
If you are completing verification or an appeal, submit your docuements directly through you student account in StudentForms. You can start by visiting Verification - StudentForms.
Uploading Documents
Use this secure upload option only if you were specifically instructed by our office.
- Documents should be scanned and uploaded as a PDF whenever possible
- Photo's are accepted if a scanner is not available (e.g., Social Security Card, High School Diploma, Passport, Permanent Resident Card), but must be clear, legible, and complete
- Signature required documents must be signed by the student.
- Files that are unclear, cut off, or are unreadable may delay processing
Please include a cover page or note with:
- Full Name
- A-Number
- A brief description of the document (example: "Identify Verification Request")
Examples of Documents You Can Upload
- Government-issued ID (driver’s license, passport)
- Social Security card or SSN documentation
- Birth certificate
- Citizenship or immigration documents
- Third-party or institutional requests (when instructed by our office)
Important Reminders
- Keep your email, phone, and address updated in your student account
- Check your USU email regularly for updates or requests
- Don't forget to set up delegates
- Processing times may vary depending on volume
Notarized Documents
Notarized forms cannot be uploaded online. Please mail them to:
