Class Waiver

By selecting the box below, I hereby recognize and acknowledge that the Employee Wellness Program of Utah State University does not carry special health and/or accident insurance that would protect me in the event of accidental injury while participating in any program or facility offered by the Employee Wellness Program.  Such insurance coverage, if any, is my sole responsibility.  I further recognize that participation in all such activities and facilities is voluntary on my part and that there are certain inherent risks which I hereby assume, and in consideration of my acceptance as a participant in such activities and facilities, I hereby release and discharge Utah State University, its governing board, officers, faculty, staff and employees for all obligations, liabilities, claims, demands, costs, and expenses, including attorney’s fees, arising out of, or in any connection with, any bodily injury sustained by me.  I similarly authorize the employees of the Campus Recreation and Health, Physical Education, and Recreation Departments and the Employee Wellness Program to notify emergency help in case of accident or injury to myself while participating in any program or facility of Utah State University if said employees determine the need to do so.  I agree to personally assume all expenses occurring from, arising out of, or connected with this medical assistance.
