R401 Forms in Coursedog Curriculum Management

  1. Log into Coursedog (Curriculum Management) at coursedog.usu.edu (SSO and Coursedog Curriculum user access required)
  2. Select Campus from the left sidebar
  3. Select the Document Type that relates to your proposal:
    • R401 New Administrative Unit Form – Use this form to request approval of a new department, college, center, or institute.
    • R401 Full Program Proposal
      • Use this form to request approval of a new associate, baccalaureate, or graduate degree.
      • Significant cumulative changes (50% or more) to an existing program require a full program proposal.
    • R401 Abbreviated Program Form – Use this form to request approval for:
      • New minor
      • New emphasis or specialization within an existing program
      • Program name change
      • Program restructure (with cumulative changes less than 50%)
      • Program discontinuation or suspension
      • Program reinstatement
    • R401 Request for Approval of a New Certificate Program – Use this form to request approval of a new certificate (academic or technical).
  4. Select + Propose New Document
  5. The “Document Type” dropdown will auto-populate with your previous selection
    • If you need to change the Document Type, cancel this request and return to the “Select Document Type” screen.
  6. Click Submit
  7. Input the required information in the form fields
  8. Click Submit proposal once ready (your proposal will be automatically saved in the DRAFTS folder until submission).

Additional Notes

  • Once the request is approved, the document will appear in the Campus Documents view (Curriculum > Campus > Select Document Type).
