Steps to Complete Onboarding
There are four main parts to onboarding for the UCC. To get you started, the first 2 parts are described below. Steps 3 and 4 are described on the following page. Onboarding tasks must be completed by the due dates provided in email correspondence with your program coordinator. You will not be unable to participate in the UCC program if you fail to complete these tasks.
The steps you will need to complete are highlighted in yellow. You may refer to this Onboarding Process Chart to track your progress. Please email your program coordinator for any questions.
Part 1 - Getting Started
This stage gathers the information to begin your onboarding process. Our questionnaire also collects emergency contacts and information about any medical conditions to help ensure your safety.
- Begin by filling out the Onboarding Questionnaire https://redcap.link/UCC_onboarding2025
- You will need to upload a photo of your ID with the Onboarding Questionnaire
- Download, complete, and then upload W-4, Direct Deposit, and photo release forms, then at the top right of this page, then upload them through the submission portal above.
- Don’t forget to include your SSN #, signature, and date when submitting these forms.
- Upload these forms through our secure portal to protect your personal information!

Part 2 – Background Checks
AmeriCorps requires that all members pass a National Security Criminal History Check.
- Once Part 1 has been completed, the program coordinator will submit a request via True Screen.
- You will receive an email from Truescreen Application Station. This email will contain instructions on how to complete your background check, which includes information on how to schedule fingerprints.
- Once your background checks have cleared, you will receive an email with instructions for the rest of your onboarding.
