Health Insurance

Utah State University (USU) requires all international students to have health insurance for themselves and their dependents (wife, husband, and/or children). USU has specific requirements for the insurance coverage of international students.

You will automatically be enrolled in USU's student health insurance when you register for classes. To waive this student health insurance charge, visit https://studentcenter.uhcsr.com/usu.

This must be done immediately every semester.

Beginning May 15, 2015, according to regulation 22 CFR 62.14, all J-1 visa holders will be required to have the health insurance coverage listed below:

Immunizations

USU recommends the following immunization. You may wish to bring a record of your immunization records with you when you travel to the U.S. and to USU.

  • Tetanus, Diphtheria, Pertussis (TDaP) – series and current booster
  • Measles (MMR) – 2 doses
  • Polio – 3 doses
  • Chickenpox (Varicella) – immunity or record of illness
  • Tuberculosis (TB) skin test – Test after foreign travel or residence outside the U.S. for more than 8 weeks
  • Hepatitis B – 3 dose series
  • Hepatitis A – 2 dose series
  • Meningitis – 1 dose every 10 years
  • Influenza – annual vaccination
  • HPV – 3 dose series
  • COVID-19
