How to Report Information About Sexual Misconduct

USU prohibits sexual misconduct. Sexual misconduct includes sexual harassment, relationship violence, sexual assault, and sex-based stalking. 

You are encouraged to report incidents of sexual misconduct to the USU Title IX Coordinator at any time at the Civil Rights & Title IX Office website. Reports may also be made during business hours by calling 435-797-1266, visiting the Civil Rights & Title IX Office in Old Main Room 405 (Logan campus), or via email at titleix@usu.edu.

When you report, the Civil Rights & Title IX Office will provide information about supportive measures, reporting options, and the investigation process

Sexual misconduct is reported to the Civil Rights and Title IX office, a non-confidential office that oversees supportive measures and the university-driven investigative process.

USU prohibits Sexual misconduct which includes sexual harassment, relationship violence, sexual assault, and sex-based stalking. All employees of USU, with the exception of SSAVI and CAPS, must report sexual misconduct. Anyone is encouraged to report incidents of sexual misconduct to the USU Title IX Coordinator at any time at the Civil Rights & Title IX Office website.
