Utah State University uses on-line reporting for many student needs. In the event of an emergency, please call 9-1-1.
Report a Concern or an Incident
Our goal is to keep our campus community safe and well. If you are concerned about an incident or a student, please know that anyone may report a concern about a USU student. Listed below are a number of different reports for different topics.
If you would like to first consult with a staff member before submitting a report, please call the Office of the Vice President for Student Affairs, at 435-797-3137 for a student of concern or for a conduct violation. For consultations regarding Sexual Misconduct, or Discrimination or Harassment, please call the Office of Equity at 435-797-1266.
Student Misconduct
Health and Safety Violation
This form should be used to report an incident involving a Health and Safety Violation. Thank you for doing your part to help keep Utah State University safe. The information you provide will be used to identify safety concerns and issues, such as failure to follow health directives, inappropriate use of safety equipment, etc, that could pose a threat to the health and safety of our students, faculty, staff and environment.
Conduct Violation
This form can be used to report general student misconduct or Student Code violations, including violations of USU Housing and Residence Life Policies. If you are unsure what type of student conduct or Code violation has been committed you may use this form to report any incidents.
Academic Integrity Violation
This form is used to report academic misconduct such as cheating, falsification, and plagiarism as defined in the Student Code - Section VI. This Academic Integrity Violation Form (AIVF) provides guidance to instructors and students, ensures minimum due process requirements are met, and allows tracking of repeat offenders at the University level.
Sexual Misconduct
This form should be used by any person to report sexual misconduct to the Title IX Coordinator. Sexual Misconduct includes sexual harassment, sexual assault, relationship violence and sex-based stalking. This report form can be utilized by an individual wishing to file a report on their own behalf or on behalf of another person who has experienced Sexual Misconduct.
Other Misconduct
Discrimination/Harassment
This form is used to report discrimination or harassment based on protected categories such as race, color, religion, sex/gender, national origin, age, disability, veteran status, sexual misconduct (including harassment), pregnancy, sexual orientation or gender identity/expression, genetic information, or retaliation.

Student Well-Being / Academic Concern
Student of Concern
Use this form to report a student’s behavior that rises to a level of concern that may lead to disruption of classroom or University activity, or that causes concern of threat to oneself or others. Remember, in an emergency, please dial 9-1-1.
Student Academic Achievement Alert
This form is used by USU faculty, advisors, and staff to report student academic performance and engagement concerns. If reasonable efforts to resolve issues and provide outreach to students have failed, the Student Academic Achievement Alert form is the next step to provide support. The Office of Academic Belonging & Learning Excellence will oversee this form and work with students to broker success.
