The USU Police Department works with the Utah Bureau of Identification (BCI) to provide Live Scan fingerprinting, physical fingerprint cards, and BCI fingerprint forms.
What Do I Need for My Appointment?
Paperwork
- Any form(s) from your employer/ lawyer/ teacher/ school/ etc.
- Any documents with details specific to your fingerprints
- Any FD-258 cards you've been provided
Government-Issued Photo ID
- Ideally your Driver's License, but can also be a passport or other current form of government ID that has your photo on it
- Please make sure it is not expired
Payment
- We accept major credit cards, debit cards, cash, check, and Apple Pay/Google Pay, except for the following:
- We do not accept American Express cards
- We do not accept any bills over $20
- We do not accept payments over the phone
Social Security Number
- You do not need to bring the physical card as long as you know your full SSN
- Unnecessary if you are getting a driving privilege card
Frequently Asked Questions
You will need to know your full Social Security Number and your CACTUS ID number. If you are doing this for your Education class, you will need to complete every task on your USIMS account, including the Educator Ethics Review, before submitting your fingerprints.
You will need to bring a government-issued photo ID that is not expired and it will cost $35.00. We will provide the photo and the sealed envelope that you will take with you to your driving test at the Department of Motor Vehicles (DMV).
Not typically since we operate by appointments only, which can be scheduled here. If we do accept a walk-in appointment, there's an additional $5 charge and it depends entirely on whether there is an opening in the schedule that day.
No, we do not process background checks or provide any results from background checks. We only provide the fingerprinting service.
No, if you are getting any physical copies of your fingerprints, you will be responsible for submitting them. If you have the paperwork for an electronic submission, then yes, your fingerprints will be digitally submitted straight to the BCI from our system.
Your fingerprint forms/ files are only valid for 30 days from when they are first captured. If you are within that time frame, then yes, we can print another set of FD-258 cards for you. It costs $5.00. Please note that they will still expire at the 30-day mark from the original day they were captured.
We offer live scan fingerprints and ink fingerprints.
Live scans can be used for:
- Any electronic submissions to the BCI (if you have paperwork that requires this type of submission)
- Printed FD-258 cards
- Driving Privilege Cards
Ink fingerprints can be used for:
- Expungement forms
- State criminal history background forms
- FD-258 cards (if specifically stated it needs to be done in ink)
Live Scan is an electronic, inkless method to capture fingerprints. It is more accurate than using ink as we are able to retake any print to ensure a clear picture. These electronically captured fingerprints can either be printed out onto FD-258 cards or can be submitted directly to the BCI. If we are submitting directly to the BCI, then the customer must have the corresponding paperwork for that electronic submission.
