Report a Concern or an Incident
Everyone plays a role in promoting a safe, respectful, and caring campus community. Student Affairs encourages anyone to report a concern about a USU student using the option below that best matches the concern.
If you would like to consult with a staff member before submitting a report, please call the Office of the Vice President for Student Affairs, at 435-797-3137.
For consultations regarding sexual misconduct or discrimination or harassment, please call the Office of Civil Rights & Title IX at 435-797-1266.
General Student Misconduct
Report student misconduct or violations of the Student Code or Housing and Residence Life policies. If you are unsure what type of student conduct or Code violation has been committed, use this form to report any incidents.

Report cheating, falsification, and plagiarism. This reporting form provides guidance to instructors and students, ensures minimum due process requirements are met, and allows tracking of repeat offenders at the university level.

Student of Concern
Use this form to refer a student to the CARE Office / Student of Concern when you are concerned a student may be a danger to themselves, when their behavior is concerning to others, or when a student is experiencing a significant extenuating circumstance and would benefit from resources. If you are concerned that a student may have difficulty getting connected to help on their own, the CARE Office can reach out and offer assistance getting connected with support both on-campus and in their community.

Student Academic Referral
For USU faculty, advisors, and staff to report student academic performance and engagement concerns. If reasonable efforts to resolve issues and provide outreach to students have failed, this form is the next step to provide support. Reports go to The Center for Academic Belonging & Learning Excellence.

Threat Assessment Management Team
The Threat Assessment Management Team (TAMT) actively collaborates with the campus community to support the recognition of and response to concerning or threatening behaviors.
