Digital Accessibility Complaint Form

Use the Digital Accessibility Complaint Form to file a formal complaint regarding a failure to provide accessible digital technology, systems, or content under USU Policy 5208. 

Formal complaints will be assigned to an investigator, who will conduct a prompt and equitable review. Following the investigation, the individual who filed the complaint will receive a written response summarizing the findings and, where applicable, proposed steps for resolution. 

The individual filing the formal complaint has the right to appeal the findings of the initial review to the Digital Accessibility Steering Committee for further consideration. 

This form is intended for formal accessibility complaints under USU Policy 5208. For minor issues, quick fixes, general feedback, or for immediate assistance with access, please use our informal Accessibility Concern Form or contact accessibility@usu.edu.
